Nurse Ratched, One Flew Over the Cuckoo's Nest (1975)
She may only be the head nurse, but make no mistake -- Ratched (Louise Fletcher) is the one running this asylum. In her hands, lobotomies and electro-convulsive treatments aren't healing therapies, they're punishments to reinforce her sovereign power over the ward. Go on, ask for a second opinion. She dares you.

Dr. Buddy Rydell, Anger Management (2003)
Jack Nicholson here takes on the role of the shrink -- a fitting role reversal, since he suffered at the hands of Nurse Ratched so many years before. His methods, less bloody but just as brutal, include sleeping naked with his male patients, picking fights with their bosses, and stealing their girlfriends -- and somehow this all meant to make you less angry. Obviously.
